What Is the Dental Wellness Membership Plan?
The Dental Wellness Membership Plan is an in-house program that provides professional dental care directly through our practice, without relying on traditional insurance. For one predictable monthly or annual payment, members receive routine preventive care along with discounted pricing on restorative and cosmetic treatments, including advanced options like Invisalign or EmFace that traditional insurance plans often do not cover.
This program is built for patients who want consistent, high-quality dental care without the complexity, denials, and restrictions that so often come with insurance carriers.
Membership Tiers & Benefits
We offer four membership tiers, each designed around different oral health needs:
Adult Wellness (Ages 13+)
Two professional cleanings, two doctor exams, one set of bitewing X-rays, one emergency exam, and two fluoride treatments per year, plus 15% off restorative treatment and 10% off cosmetic treatment.
Child Wellness (Under 13)
The same preventive schedule as the Adult plan, with 15% off restorative treatment.
Perio Wellness
Initial Periodontal Therapy Plan
Initial periodontal therapy plus three additional cleanings in the first year, two doctor exams, a full mouth X-ray series, one emergency exam, four fluoride treatments per year, and the same restorative and cosmetic discounts.

Program Terms & Conditions
Enrollment and Payments
Membership requires a valid credit card on file for automatic monthly or annual billing. Debit cards cannot be used to guarantee recurring credit programs. Plans are valid for 12 months from the date of enrollment and will renew automatically unless 30-day written notice is provided.
Usage and Limitations
This is a wellness membership plan, not dental insurance, and cannot be combined with outside insurance, other office promotions, or third-party financing such as CareCredit. A $75 fee applies for any appointment missed or canceled with less than 24 hours' notice. A one-time administrative setup fee of $1 may apply upon initial enrollment.
Patient Informed Consent
By enrolling, you acknowledge that you have read and agree to the terms of the In-House Dental Wellness Membership Plan, and you authorize the practice to automatically bill your credit card for membership fees and any applicable missed-appointment fees.
